If your student is being referred for an Emotional Disability, these forms will help us during the referral and evaluation process.
We must have your permission prior to starting the referral process for your student. After we have your permission, we will start working on the referral with you and the teachers. Once we have established interventions and they have been in place for an adequate amount of time without progression, a referral will be made to have an educational evaluation.
